Causes
Peyronie's disease
This is a serious condition where the internal
tissue of the penis turns to scar tissue. This disease was
first documented by a French physician named Francois de la
Peyronie, whom had discovered that scar tissue in the penis
could result in significant bending of the penis.
Because this scar tissue is hard and inflexible,
it causes the penis to bend when erect. A plaque on the top
of the shaft (most common) causes the penis to bend upward;
a plaque on the underside causes it to bend downward. In some
cases, the plaque develops on both top and bottom, leading
to indentation and shortening of the penis. Although this
curvature can sometimes by quite painful, the plaque itself
is non-cancerous.
It is also possible that the penis may be hard at the base,
but soft at the top or skinny in the middle like an invisible
band has been fastened around it.
Unlike hypospadias, which is an accident at birth, Peyroie’s
disease is acquired during adult life. It can occur fast,
without any apparent reason; a man may wake up one morning
with his erection bent over. It affects about 1 % of men.
Most reported cases have been white males in the 40-60 age
range.
Weak and poor development
Weak and poor development of the corpora cavernosa
(Spongy tissue) is another cause of penis curvature. The penis
consists of spongy tissue called the corpora cavernosa. If
the little openings in the spongy tissue on one side of the
penis develop to a lesser degree than that on the other side
then the lesser-developed side will have stronger blood circulation.
This increased blood flow would force the penis head over
the furthest point of leverage, causing a curved or banana
shaped penis.
Restrictive clothing
When boys who wear tight underwear get erections,
their penises are restricted from a normal outward growth.
They often bend their penises over to the side to hide the
embarrassing erection. This affects the development of the
young penis because blood will flow better through one side
of the spongy tissue and leave the other side under nourished.
Injury
Acute or repetitive trauma (hitting or bending)
can cause localized bleeding inside the penis. This leads
to fibrin deposition (scar tissue) in the tissue space that
receives injury. Also, the trauma can cause collagen to be
trapped that can lead to pathologic fibrosis – the formation
of calcium deposits.
Age
With age there is a decrease in the elasticity
of collagen fibers which can also lead to scar tissue.
Other causes
Vasoactive substance injection therapy, vitamin
E deficiency, the use of beta-blocking agents and autoimmune
phenomena.
