Answer:

Your Cortisol Morning (saliva) and Cortisol Evening (saliva) show you have a high level of anxiety due to the excessive adrenal medulla function. Your TSH is too low, your T4 is too high. Therefore, you have hyperthyroidism. This problem can cause a sympathetic fight-or-flight response during sex, placing your body in a state of stress. All of this can cause premature ejaculation.

I think your high cortisol and DHT levels are driven by hyperthyroidism. This leads to overproduction of the inflammatory hormone prostaglandin E2, resulting in sexual urgency and persistence sexual arousal. Thus, your high DHT level is likely to over-stimulate and inflame your prostate, resulting in premature ejaculation.

In addition, a hypothyroid condition (high TSH and low T4) can cause a deficiency of testosterone and DHT. This, combined with excessive prostaglandin E2, will result in a weak erection and premature ejaculation. At the same time, a low level of testosterone and DHT will increase prostaglandin E2 production in the prostate, seminal vesicles and testicles in an attempt to stimulate the testicular function and seminal production. The resulting semen is very watery and thin.

A weak erection is caused by insufficient blood flow to the penile sponge-like tissue or when the tissue is incapable of holding blood. When there is insufficient blood flow, the penis cannot fully expand. There are a variety of factors that can cause arteries and veins to constrict and reduce blood flow. Common causes are smoking, poor diet, penile injury, some medications, drug or alcohol abuse, or psychological factors. A weak erection can also lead to premature ejaculation. When a man experiences a weak erection, he may contract the muscles around his genitals in order to aid the erection. By doing so, he subjects the prostate gland to added pressure and stress. This added load on the prostate can mimic orgasmic contractions and precum will lubricate the urethra, leading to premature ejaculation.
