I am a 32-year-old male. I have been taking SSRI for a long time; my last medication was T?frenil. The last medication was supposed to stabilize my serotonin ,dopamine, and norepinerine levels in my brain. I have problems almost in all aspects of sex. Sustaining an erection and premature ejaculation. My question is: shouldn't the medication help me since it’s reestablishing my neurochemical balance. I don't understand, it seems like my sympathetic system is in overdrive but shouldn't the medication normalize this? My psychiatrist is taking me off from T?frenil and changing me to Wellb?trin but what about my Sympathetic system. Please advice me. Should I go to a professional about this or should I go to an urologist or an endocrinologist? HELP PLEASE. I feel like a mess!

Boy! That is a mess! The drug won't balance your brain's neurotransmitters serotonin, dopamine and norepinephrine levels. It just blocks the re-uptaking of serotonin and norepinephrine and don't increase the biosynthesis of the neurotransmitters. It produces natural unbalance for the biosynthesis of the brain's neurotransmitters. You have been hooked up by the SSRIs. It is not easily for a patient to get rid of it. This is not funny at all.
Once serotonin is trapped in the interneurons, the action of the neurotransmitter acetylcholine in the parasympathetic and sympathetic nerves is partially blocked. The parasympathetic nerves are responsible for energy restoration, healing and erection. If the parasympathetic nervous functions are insufficient, the brain will shut down its action on the penis. As a result, you experience weak erection or impotency.
The norepinephrine-reuptake inhibitor is to trigger the sympathetic nervous function for acceleration of orgasm or ejaculation regardless of the penile or clitoral/G-spot erections, which are driven by the parasympathetic nervous functions.
Now you are switching to the dopamine-reuptake inhibitor which will increase your libido and accelerate orgasm (premature ejaculation), but won't help your erection unless you block the sympathetic alpha-2 receptors with an alpha-blocker. Generally, Norepinephrine and Dopamine-reuptake inhibitors will promote sympathetic "Fight or Flight" functions, which mean " Premature Ejaculation or Erection Withdrawal " for sex, if your serotonin level is too low. But, anyway, without powerful brain acetylcholine and parasympathetic nervous functions, you won't get a good erection for sex no matter what! Blocking the sympathetic alpha-2 receptors is insufficient for erection if the parasympathetic nervous function quits! The acetylcholine/parasympathetic nervous functions are "Yin Chi"; the Dopamine/Norepinephrine nervous functions are the "Yang Chi"; the Epinephrine nervous functions are the "Wei (Defense) Chi". Without Yin to support your internal organ function, Yang and Wei cannot stand - your body will catch "Fire"!
